I certify that the information given on this form is correct. I understand that any wilful omission or falsification may lead to the disqualification of this application for a place on the programme. I understand that there is a level of expectation while attending the sessions in terms of athlete commitment, attitude and work rate, and if these fall below the expected standard then the athlete may be asked to leave the programme with no refund. 

I understand that the Northumberland and Durham Swimming Association or any of their staff are not responsible for any loss or injury during their sessions.

The Northumberland and Durham Swimming Association reserves the right to remove from athletes from their rogrammes, without refund, anyone who is found to be bullying, misbehaving or displaying disruptive behaviour.

You must declare all medical conditions during registration and keep us up-to-date should this change.

Your booking is not confirmed until payment has been received. Paid spaces will be given priority.
